Summit Primary Care
Superior Care for the Entire Family:
Infants to Seniors
Scheduling the appropriate amount of time for your specific medical problem is very important. If you have a complicated problem or multiple problems that you want your Provider to address during your visit, we recommend a several minute phone conversation with our triage staff so enough time is scheduled and sufficient notes are entered for the visit.
If you are an established SPC patient and want to schedule a routine appointment, we recommend that you use our secure, web-based Patient Portal to request the appointment. Our Portal Coordinator should respond to your request within an hour if we receive it before 4 PM, and by early the next morning if we receive it after 4 PM. If you are an existing patient and have not yet created your Portal account, please call our office to provide your email address. Once we receive that email, we will set you up an account on the portal and send you an email with a temporary password.
If you are a new patient to SPC, you can register for a portal account by clicking this link: SPC Patient Portal. Start in the section for New Patients to complete our registration forms and submit to us. When your registration is processed by our office staff, you will get an email with a temporary password, which you will be prompted to change after you log in the first time. You may then request an appointment from the portal.
Our triage staff is available to take your calls at (615) 883-2331 from 7:00 AM to 5:30 PM Monday through Friday. You should be prepared to briefly but completely describe the situation you want to be seen for.
For most problems, we will usually be able to schedule you the same day of your call. If your Dr. can't get you in to meet your time needs, you may be scheduled with the PA who your Physician works with. Your Physician will review your visit notes in those situations and be actively involved in a care plan even if they are unable to see you.
Thank you for choosing Summit Primary Care for your medical care.
