At Summit Primary Care, we follow Federal HIPAA regulations in regards to maintaining the privacy of protected health information. Any information you provide us through the questionnaires on this website is kept in strict confidence. Your Social Security Number and other personal information is used only to verify your status as one of our patients, and to insure that we reference the appropriate medical records when dealing with your questions and requests.

All protected health information you provide us will be released only as per the HIPAA regulations. Providing us with information via this website is strictly voluntary, and thus it is understood that the information you provide is given at will.

Your privacy is of utmost importance to us, and we pledge to exercise all due caution in maintaining the security of any communications you have with our providers and staff.
