Billing Procedures

All patients must sign our Patient Billing Agreement, which you may print by clicking here.

Summit Primary Care uses online insurance verification systems to determine your insurance plan eligibility, co-pay amount, co-insurance amount, and your year-to-date remaining deductible. Prior to seeing the Dr. we will request that you pay your co-pay amount and any outstanding "patient due" balances you may have remaining from previous visits. If you have a large remaining deductible, we will ask you to make a deposit for the estimated office visit charges.

After your office visit, we will file the bill with your primary insurance. Your primary insurance company will respond by indicating how much of the charges are your responsibility to pay. Any part of that amount that you did not pay at the time of the visit will go on your account balance. You can log-in to our Patient Portal and pay it by credit card or ACH check or call us to make payment over the phone. If you do have an account balance after we are notified by your insurance, we will send you a monthly statement that month and one the following month, unless your patient balance is paid in full. If full payment of your account balance is not received within 4 weeks after you are sent your second monthly statement, we will turn over your overdue account balance to our collection agency. You can also create a payment schedule using your credit card or ACH bank drafts to avoid collection activities. However, once your balance is in their hands, they will take over the process, and their fees will also be applied to your balance.

If you have medical insurance, we will electronically file the visit charges with your primary insurance. When we receive their response, they will tell us how much of the charges are your responsibility to pay from that visit. We will subtract from that the amount that you already paid at the time of the visit and then we will send you a bill right away for the balance. We keep sending a monthly bill to you each month for 2 months unless your patient balance is paid in full before then. If you do not pay off your balance by the third month, we will turn over your overdue account balance to a collection agency.

Please understand that we want you to use our services, but we cannot afford to perform services without payment in full.

New patients will need to pre-register, and your portal account will be active once your registration is accepted by our office staff.
